Clicincal Social Worker Associate - Program for Assertive Community Treatment

McLean Hospital(MCL) – Belmont, MA

To work as a member of an interdisciplinary treatment team to provide person-centered care to clients and their families who are participants of McLean Hospitals PACT team with goal of maintaining psychiatric stability and building independence and life outside of traditional treatment. The PACT team serves adults who have primary diagnosis of bipolar disorder or psychotic disorders, with some individuals who have co-occurring substance use or personality disorders, etc. The clinical social worker will provide case management, vocational support, daily organizational skills, community integration, family support and coaching, and additional psychosocial rehabilitation. They assist clients to obtain and maintain structure and achieve their goals. They will provide individual interpersonal/ADL/coping skills coaching (often occurs in-vivo in the community), individual therapy/ family therapy on case by case basis, and psychoeducation to client and family. Services are most often provided in a clients home or community. This person serves as a primary clinician on the PACT team for set of clients assigned to them, while also working with other PACT clients based on client and team need. This role includes regular clinical assessment and crisis intervention as part of on-call rotation.
JOB RESPONSIBILITIES:Specific Responsibilities/TasksPACT

  • Serve as a primary/lead clinician for a subset of PACT clients and their families while also being involved in other PACT cases based on the needs of the treatment team and client
  • Provide individual, group, and family psychoeducation and coaching utilizing a range of models and approaches
  • Provides interventions and services in-home and community based
  • Provides case management including treatment referrals, assistance with obtaining benefits, vocational support, help establishing routine and obtaining structure
  • Provides care coordination with community agencies, other treatment providers or levels of care, family members and supports
  • Provides regular contact with family members including family meetings, phone coaching, and psychoeducation
  • Participates in daily interdisciplinary rounds and weekly staff meetings
  • Communicating clearly and effectively with an interdisciplinary team to provide comprehensive and integrative care, works effectively with other team members
  • Provides regular clinical assessment of symptoms, diagnosis and clinical formulation
  • Assists in the development of person-centered treatment plans based on clients strengths, needs, abilities, and goals and helps clients achieve their goals
  • Completes clinical documentation appropriate to services provided including intakes, treatment plans, individual therapy notes, detach notes
  • Other duties as required
  • Each member of the PACT team is required to be on on-call rotation

  • QualificationsJOB QUALIFICATIONS:
    Education: MSW
    License:LICSW preferred
    Attaining LCSW licensure is not required upon hire to perform the essential functions of this job, however, as a condition of continued employment, it is a requirement to receive LCSW licensure within 12 months of hire, or upon the one year anniversary of employment-along with the annual review. Failure to attain this licensure will result in termination of employment-failing to meet a condition of employment.Exceptions may be made for extenuating circumstances preventing an employee from attaining licensure must be approved by the Director of Social Work

    Skills:

    Self-motivated individual who has the ability to work collaboratively with the interdisciplinary team and community based organizations as well as self-motivated and is able to work independently. This individual needs to have good organizational skills, the ability to multitask, and the ability to be flexible. One must have good judgement and assessment skills in crisis situation. One must be able to manage irregular or extended hours in order to ensure a safe/therapeutic-staffing pattern. Energy level must be sufficient to accomplish tasks under stress, maintain appropriate professional demeanor and maintain safety when interacting with clients. One must have the stamina, energy, and motivation to perform various tasks and interventions with clients in the community. Must have experience with assessment, diagnosis, and appropriate documentation.
    Individuals applying for this position must have clinical expertise in the area of bipolar and psychotic disorders including direct care with both patients and families.
    Working Conditions:The PACT team offices will be based on the McLean Campus in the Admissions Building, however primary clinical work will be conducted in the community.
    Must have a current and valid drivers license and a registered vehicle that has passed yearly inspection. All employees must posses basic computer skills to use a variety of electronic or online systems for communication, clinical and administrative purposes.
