ISS Case Manager, Per Diem

  • Provide support and/or training in developing and/or accomplishing personal goals.
  • Conscientiously implement the principle of well-being so that our individuals are safe, enjoy optimal health and economic resources.


  • Coordinate Support Services for individuals which include: skills training and money management.
  • Assist in accessing Personal Care Attendant, VNA services, therapy services, health care providers and transportation as needed.
  • Assist individuals in understanding and making choices regarding medical, SSI, and financial requests.
  • Participate in the development of ISP emphasizing the empowerment of the individual.
  • Oversee the daily implementation of the ISP and ensure that services are delivered in accordance with the Plan, review on a quarterly basis with the individual and assist the individual with revisions.
  • Assist individuals with accessing community resources in the areas of health care, housing, employment, recreation, transportation, adult education, and independent living skills.
  • Have a working knowledge of Program Policies and Procedures.
  • Respect the individuals right to make choices, take risks, and maintain confidentiality.
  • Encourage and assist participants with development of relationships with other community members.
  • Attend regularly scheduled staff meetings and supervision.
  • Assist individuals with self-advocacy.
  • Record data and write monthly and quarterly progress notes, maintain case files in accordance with DDS regulations.
  • Report to supervisor any issues or concerns regarding individuals on their caseload.
  • Consult with nursing staff on all medical issues, needs, and concerns.
  • Be available to accommodate reasonable agency-wide staffing requests in times of need.
  • Attend required trainings to increase knowledge and skills related to the services being provided.

Essential Physical Requirements:

The ability to move independently throughout a wide range of environments, some of which may not be wheelchair or otherwise handicapped accessible, is required. Independent transportation to locations that may not be served by public transportation may be necessary. Fidelity House Human Services will make reasonable accommodations, whenever possible, when needed and/or requested by employees. Requests for accommodations will be honored, unless the accommodation would cause undue hardship on the operation of the program and/or on the organization.

  • High School Diploma or equivalent.
  • 1 year experience working with the developmentally disabled population.
  • Must have a valid drivers license and access to a car
  • Reading, Writing and Math skills
