Job Description Summary: Community Health Center, Inc. (CHC) is one of the countryâ€™s most creative and dynamic providers of primary medical, dental, and behavioral health services, and a leader in practice-based research, health professionals training, and use of innovative technologies to advance health and healthcare. CHC is designated as a federally qualified health center and a patient-centered medical home by HRSA, the Joint Commission, and NCQA, respectively. We deliver more than 500,000 patient visits per year from primary care hubs and community clinics across the state of CT, all connected by technology and common standards for quality. We employ several hundred medical, dental, and behavioral health providers who are engaged in practice, teaching, and research. Our Weitzman Institute is devoted to research and practice transformation and is recognized around the country as one of the premier research institutes focused on improving health care and health outcomes for special and vulnerable populations. In addition, the organization has developed three wholly owned subsidiaries from the original pilot developments within the Weitzman Institute: the National Nurse Practitioner Residency and Fellowship Training Consortium (NNPRFTC), the National Institute for Medical Assistant Advancement, and the Community eConsult Network. Job Description: The Outreach and Eligibility Associate (OEA) acts as a member of a team focused on supporting our uninsured and under insured patients at CHC. The OEA is committed to advocacy and creating resource connections in the communities served. In addition to helping individuals and families, the OEA collaborates with all CHC departments and programs to resolve eligibility issues, as well as breaking down access barriers related to social determinants of health. The OEAâ€™s responsibilities are broad in scope, ranging from direct patient support to leading special projects requiring work with large groups. This role is based at the health center, as well as in the community. ROLES AND RESPONSIBILITIES Performs outreach for the agency, as needed Refers patient and family to community resources, providing access to services such as financial assistance, housing, and other support services, as needed. Interpret and explain information such as eligibility requirements, application details, payment methods, etc. Compile, record, and evaluate personal and financial data to verify completeness and accuracy, and to determine eligibility status. Answer applicants' questions about enrollments and benefits. Interview and research opportunities for public assistance while gathering information pertinent to their applications, for example food stamps/husky, etc.. Partner with communications to modify and distribute educational materials and programs for community agencies, local government, and state government. Document activities and record information, such as the numbers of applications completed and persons assisted. Submit reports and review reports or problems with manager. Consult with Manager concerning programs for individual families. Advocate for patients to resolve social determinants of health Provides feedback to referring clinical or clerical personnel referring clients for assistance. Maintains computer based log of all eligibility activity within our EHR. QUALIFICATIONS A minimum of one year experience in human service field, required. One year community based outreach experience, required. Associates degree, required Bachelorâ€™s degree, preferred Bilingual required; dependent of community served. Thoroughly familiar with all public and private assistance programs in area of population served. Electronic Health Records experience, preferred. PHYSICAL REQUIREMENTS/WORK ENVIRONMENT Travel required, must have reliable transportation Combination of sustained periods of sitting at keyboard, walking and stair climbing Minimal lifting and carrying activities COMMUNICATION SKILLS Excellent oral and written communication skills are required. This position must interface with patients, family members, community agencies and other providers within the organization. CONFIDENTIALITY Personal Health information is kept in strict confidence. Confidentiality of patient and business information is a requirement. Full access to patient medical records and encounter data. Confidentiality must be maintained according to CHC policies.
