Primary Case Coordinator (Group Facilitator)

We are seeking someone who is organized, creative, energetic, and able to think fast on their feet in a very fluid environment. As a Primary Case Coordinator (Group Facilitator) you will plan, develop, and facilitate group activities on a variety of subjects related to intellectual/developmental disabilities, mental health, life skills, etc. Adaptive communication skills will help you work with a wide variety of people of varying abilities.

This is an entry-level healthcare position.

Your typical responsibilities may include:

  • Ensuring individuals are safe and healthy.
  • Implementing and assisting individuals with treatment plans and goals.
  • Developing and implementing lesson plans for assigned group topics.
  • Facilitating recreational activities at the program and in the community.
  • Transportation of individuals to/from the program and in the community.
  • Approving/conducting, reporting and documenting verbal/physical behavioral interventions.
  • Documenting all group activities and services provided in an electronic system.
  • Adhere to confidentiality, and all agency policies and practices.
  • This is a Full Time, non-exempt position.

    Hours: Mon - Fri 8 am -4 pm

    Starting rate: $12 per hour

    Qualifications:

  • High School Diploma or G.E.D
  • At least 1-year experience working with children or adults with developmental disabilities or mental health (Preferred)
  • Requirements:

  • 18 years or older.
  • Pass a criminal and motor vehicle background check.
  • Valid non-probationary(provisional) drivers license.
  • Pass pre-employment physical exams and TB test prior to working.
  • Successful completion of mandatory Orientation; including American Red Cross First Aid/CPR, Medications, Preventing Abuse and Neglect, Crisis Management and more.
  • Additional onsite training is conducted and offsite is supported and encouraged.
