Social Work Practitioner

The Pediatric HIV case manager/social worker?s main responsibilities will be to provide psychosocial counseling, education and case management services to HIV infected children, adolescents, young adults and families on issues of illness and disease progression, medication adherence, grief and loss, sexual health, disclosure of HIV diagnosis, pregnancy planning and legal needs. Provide prenatal services and follow up care to HIV infected pregnant women and their newborn babies. Provide occasional home visits as needed. Complete psychosocial assessments and develop treatment plans, inform physicians of case status, document all social work services in the medical record, make referrals to community agencies and programs, and monitor clinic attendance, medication compliance and transportation needs. Provide education and training on the psychosocial aspects of Pediatric HIV to caregivers, medical professionals, and community agencies, as needed. Organize and facilitate monthly interdepartmental HIV maternal/infant care meetings. Complete and maintain medical database(s) on HIV patients. Collect and maintain data on all patient care and services to assist in formulating reports for the Ryan White grant. Document contacts in Careware as required by funding source. The individual will report abuse, neglect and dependency concerns to the Department of Social Services and follow-up with documentation and phone calls with progress of the case. He/She will coordinate with other Pediatric HIV Social Workers at other sites where Pediatric HIV care is being delivered, so that a uniform standard of care is provided. He/She will receive training and ongoing instruction from the Senior Social Worker at UNC, Duke Medical Center, and/or ECU which may include on-site training, psychosocial and risk assessment of HIV patients and families. He/She may participate in in-service education/group activity involving HIV care at Vidant Medical Center and ECU Brody School of Medicine. CLINICAL SUPPORT POSITION - NOT SUBJECT TO STATE HUMAN RESOURCES ACT. The ECU Brody School of Medicine is affiliated with Vidant Medical Center. Vidant Medical Center is the flagship hospital for Vidant Health and serves as the teaching hospital for the Brody School of Medicine at East Carolina University.

Minimum Qualifications:

Bachelor s degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board. All degrees must be received from appropriately accredited institutions.
