Nurse Practitioner

We provide primary care services to patients who have been confined to their homes due to age or disease. We work with geriatric populations to manage their chronic conditions with a multi-disciplinary, home-based, team approach. Our Physicians gain a sense of fulfillment and satisfaction during their practice due to the unique way they interact with patients.

Looking for Nurse Practitioners with the following qualifications:

* Nurse Practitioner degree from an accredited medical college or university

* Licensed as a Nurse Practitioner in the state of Illinois

* Valid Illinois Controlled Substance License or the ability to obtain a license before starting employment

* Valid license with the Federal DEA or eligible to obtain a license before starting employment.

The house call primary care model allows physicians to spend more time with the patient and maintain a work-life balance. Medicare's Independence at Home Demonstration Project has shown house call primary care visits to be an innovative and effective patient-centered care model which improves patient outcomes.
