Job Description :

The Providers at MD at Home provide primary care services to patients who have been confined to their home due to age or disease. We work with geriatric populations to manage their chronic conditions with a multi-disciplinary, home-based, team approach. Our provider gain a sense of fulfillment and satisfaction during their practice due to the unique way they interact with patients.

We are looking for Physician Assistant with the following qualifications:

* Physician Assistant degree from an accredited medical college or university

* Licensed as a Physician Assistant in the state of Illinois

* Valid Illinois Controlled Substance License or the ability to obtain a license prior to starting employment

* Valid license with the Federal DEA or eligible to obtain a license prior to starting employment.

The house call primary care model continues to allow providers the ability to spend more time with the patient and maintain work-life balance. Medicare's Independence at Home Demonstration Project has shown house call primary care visits to be an innovative and effective patient-centered care model which improves patient outcomes.
